LIABILITY WAIVER & REFUND POILICY
~Please Read~
In accordance with the acceptance of this application, the owner and/or handler hereby releases and agrees to hold harmless MY K9 TRAINER, it's owners, instructors, employees and agents from any liability for damages, injuries or loss, if any, suffered or sustained by the owner.handler or dogs as resulting in a personal injury or property damage accuring in connection with or during training. The registration fee paid is applicable ONLY to this class and is refundable PRIOR TO THE CLASS START DATE ONLY, on or after that date, no refunds will be given. I understand the the trainer may, at his or her discreation, refuse to admit any dog which does not apprear healthy or has an injury that, with continued participation in the class, would cause further injury to the dog. I further understand that the trainer, at his or her discreation, may dismiss and dog and/or handler from class for violating any class policy and/or safety regulations, including but not limited to and dog showing signs of aggression.
By submitting this registration form, you agree that you have read, understand and agree to be bound by the terms of this contract.
You will be required to sign a paper copy the first day of class.
